Description
Chairside, made for your clinic
The Chairside Clinic Edition is a four-week paper planner for patients in their first weeks of in-center hemodialysis, or new to your clinic. Patients note what happened at treatment, notice what stood out, prepare one clear question, and write down what the care team told them, following the Renal Care Planner method: Track → Recognize → Prepare → Participate.
Made out to your facility
At checkout, you’ll enter your facility’s name, street address, and main phone. Your files arrive with those details already printed in: on the planner cover, the welcome page, and the footer of every page, and in the staff guide’s license details along with your license period and order number. The rest of the clinic pages, like parking, visitor rules, and your care team, can be typed into the PDF once and saved, or written in by hand.
What you receive
- Patient Planner (25-page PDF): a cover with your clinic’s name, a welcome page and care team page your clinic completes once (schedule, arrival, clinic routines, emergency plan, contacts), a “My first weeks” checklist of education topics, twelve treatment-day pages, four weekly reflections, and a page that gathers each patient’s top three questions for the care-plan meeting.
- Staff Guide (4-page PDF): who it’s for, a two-minute introduction script, ways to fit it into chairside rounds and care-plan meetings, how to set up and print your clinic’s copy, and your license details.
Your license
One facility location may print and photocopy unlimited patient planners for 12 months from purchase and give them to its patients and caregivers free of charge. Copies already given to patients may be kept after the license ends. Files may not be resold, shared with other locations, or posted online. Need several locations? Contact us for multi-location licensing.
Digital download only. For patient education and personal organization; not a clinic treatment record, nursing documentation, or a substitute for professional guidance. Every treatment page reminds patients to tell staff about concerns right away.
